In order for Dr. Smith to provide a complete and comprehensive medical opinion it is important that he reviews all your medical information. Please be prepared to provide this information to our office along with your current medical insurance information. Below are links to our patient information forms. We can bill for any insurance plan as long as the patient is allowed to see a doctor of their choice. Please call us for more details.

Payment Policies

Co-payments and payments for dental services must be rendered at the time of the visit. We accept cash, check, VISA, MASTERCARD and CareCredit®.
